Our focus is to bring comprehensive therapy services to you in the comfort and privacy of your home. Unfortunately, it is not always possible for a person to stay in their residence. When a client needs to transfer to another facility, whether its temporary or permanent, we are there to assist with that process to ensure a smooth transition.

Specifically we will plan and manage the coordination of care including:
evaluate client’s needs and environmental factors to promote independent living
address community mobility, dementia management, low vision needs, fall prevention
and psychosocial wellness.

Following his stroke, in-home physical therapy has allowed my husband and I to enjoy our ‘together time’ more, because of the progress he has made in both strength and stamina. Continued in-home sessions have enabled him to increase his activities, outings and general confidence.

I have had the pleasure of working with Lucy K. in my home for six months now. She is always on-time, courteous, encouraging and creative. She knows when to push me, and when to ease off a little bit. My movement and energy have improved tremendously since we started working together. I always look forward to her twice-weekly visits.

My father has worked with two different therapists at Active In-Home Therapy. He has multiple medical issues since he had a stroke three years ago. The therapists’ knowledge and compassion never cease to amaze me. Even on his bad days, my father seems ready to get to work when his therapist arrives. Both therapists have been helpful educating both my Father’s caregiver and other family members.

In the comfortable surroundings of his own home, my aphasic husband has emerged from silence and is flourishing under the guidance of his able speech therapist.
